To enrol your child
in family day care with our Service, you must
first
request child care.
You can do
that online using our
request form,
or
by telephone at (07) 3393 4444, or in person
at
our office.
Once we determine
that we are able to place your child/ren with a Care Provider in our
Service, we promptly communicate with you, arrange for you to meet and
approve your Carer, and assist in completing required forms
including Child Care Benefit (CCB).

Once
we have approved your request for childcare, you
can proceed to 'enrol your child in care' using our secure
online forms, and you can even pay the initial $26 enrolment fee
online in a secure transaction.

BEFORE YOU BEGIN
Please note
that you will need your Centrelink Customer Reference Number
(CRN) to complete the enrolment form.
If you do
not already have a CRN, you can obtain one by calling the Family
Assistance Office at 136 150.
You must
provide your Customer Reference Number so that we can enter you
and your child into the Childcare Management System (CCMS) of
the Australian Government.
The online
form will not go forward unless the CRM is entered.
