► REQUEST CHILDCARE | Online Request Form

To enrol your child in family day care with our Service, you must first request child care.

You can do that online using our request form,
or by telephone at (07) 3393 4444, or in person
at our office .

Once we determine that we are able to place your child/ren with a Care Provider in our Service, we promptly communicate with you, arrange for you to meet and approve your Carer, and assist in completing required forms including Child Care Benefit (CCB).

Once we have approved your request for childcare, you can proceed to 'enrol your child in care' using our secure online forms, and you can even pay the initial $26 enrolment fee online in a secure transaction.

You may prefer to complete the enrolment process using one
of the other options outlined below. Either way, please plan
to pay the enrolment fee up front at the start of the process.

The 'Enrolment Pack' of Forms below can now be completed, and your Enrolment Fee paid, entirely online -- in a secure process over the internet.

Download and save the 'Enrolment Pack' forms to your PC, then print out paper copies, and hand-enter the required information (off-line);

Your personal hand-signature with date is required;

You can then post or fax the forms to us;

You can also submit high-resolution scanned images (.jpg or .gif) of your hand-signed forms to us by e-mail

Or you can bring your completed forms, with payment, to the Bayside Family Day Care Office in person.

IMPORTANT:

We do require payment of the Enrolment Fee at the time you submit your Forms. We do regret that we are unable to process your Enrolment Forms unless they are accompanied by payment of the initial $26.00 [AUD] fee.
