ABOUT THE ONLINE ENROLMENT PROCESS:

     Have we advised you that we have a place 
     for your child?

     If the answer is Yes, keep going!  If not,  
     you may wish to request childcare.

  To enrol your child with our Service, please
complete and submit these online Forms:

Mandatory

1  Enrolment Form

2  Parent / Guardian Agreement

3  Parent / Family Preferences

Mandatory but Separate

4  'Child Care Benefit advice of 
    family details'

This is an Australian Government Family Assistance Office form. 

You can download it here.

This form is not part of our online forms submission process. You will need to print, complete, and submit the form offline.  We can Fax this form on your behalf from our office if need be.

Payment of the initial Enrolment Fee of $26 is required at time of forms submission. If you wish, you can pay now, prior to completing the forms.

           

If you do not use the online payment feature, you must make immediate arrangements with our office to assure payment.  We regret that we are unable to process Enrolment forms unless this initial fee is paid.

You have the option of moving through the entire sequence
of forms and completing your payment online.

When you submit each form, you will automatically be
given 1-click access to the next form in the sequence.

'Mandatory Forms Only'

The Bayside Family Day Care Office may have told you
that you do not need to complete some forms listed above
in the 'As Required' category.

If so, simply complete the mandatory forms and proceed
to pay your Enrolment Fee using our Paypal Checkout.

You will still be able be review the 'As Required' forms to determine
whether you have information relevant to your child's care,
that you may choose to submit using an individual form.
